Plaintiff fact sheets can be long and difficult to complete and often with very short time lines. Our team of experienced nurses have years of training completing complicated health histories and intake forms and have completed numerous PFS.

PFAS

Also known as “forever chemicals”, PFAS are a group of chemicals that can remain in the body for decades and can be found in ground water in certain areas of the United States. PFAS can cause a variety of health problems, including certain cancers.

3M

Standard issue combat earplugs manufactured by 3M caused auditory injuries, including permanent hearing loss and tinnitus, in thousands of military service members. An internal records review revealed manufacturer knowledge of the earplug defects, though no action was taken to correct the problem or inform the consumer.

Camp Lejeune

The military base’s drinking water was contaminated with VOC’s (volatile organic compounds), which the CDC has linked to severe and deadly health effects, including cancers, birth defects, childhood diseases, and more. Though the military was fully aware of the exposure risks, military service members and their families who consumed the water were not notified of the exposure until many years later.

NEC (Necrotizing Enterocolitis)

Infant formula manufacturers marketed bovine based infant formula as being equal to breast milk, omitting information about this type of formula being strongly linked to infants developing NEC, especially premature infants.

TIVAD (Totally Implantable Venous Access Devices)

The FDA has recalled multiple types of TIVAD devices. TIVAD devices can result in serious complications, including blood clots, blockage, other serious complications, and death.  

Roundup

Exposure to the common weed killer, used both commercially and residentially, has been linked to certain forms of cancer, including Non-Hodkins Lymphoma, B-Cell Lymphoma, and Leukemia. The manufacturer now faces multi-district litigation across the country.

Hernia Mesh

The manufacturer recalled their Hernia Mesh product in 2016 after it was determined to cause serious health problems. The company now faces litigation.

Exactech

Manufacturer Exactech has had multiple recalls of it’s polyethylene hip, knee, and ankle replacements. The replacements have caused injury due to material breakdown, bone loss, infection, and other complications, leading to the need for revision surgeries and occasionally amputations. 

Breast Implants

Recalled breast implants have been found to cause bruising, skin thinning, lymphedema, lymphoma, and other complications. Ruptures, bleeding, and infection can also occur.

Chemical Hair Relaxer

Chemical hair relaxers contain toxic chemicals that can result in uterine cancer, ovarian cancer, uterine fibroids, and other reproductive complications.

Talc

Talc baby powder was marketed and sold for decades, even though the company was aware that this product contained asbestos, which causes ovarian cancer and mesothelioma. The company took no action to warn consumers about the risks.

My paralegals/social workers/mitigators do my records review. Why do I need you?

While we know that paralegals are amazing at multitasking and are absolutely invaluable, they are not classically trained in medical terminology or the medical field. By allowing us to assist with medical record review, your paralegals can help get you more cases to grow your business!

Why can’t I do my own medical record review?

You can, but we don’t recommend it. Why? We have years of medical expertise and know how to quickly identify mechanisms of injury, missing records, related injuries, and nuances in the record that need to be investigated. Plus, this takes away valuable time that you could be using on other aspects of the case.
